ADK Hospital Patient Safety Awareness Week – 2019

Patient Safety Awareness Week is an annual recognition event intended to encourage our staff and patients to learn more about health care safety. 

This Week started on 10th March and ended on 16th. ADK Hospital Patient Safety Committee had organized variety of activities to celebrate this week. It was an opportunity to recognize the importance of quality care and patient safety throughout the hospital and use leading practices to ensure consistent delivery of care to patients, when they need it most.

Patient Safety Awareness Week served as a dedicated time and platform for growing awareness about patient safety and recognizing the work already being done.

Patient Safety Awareness Week Activities

There were several activities conducted in recognition of Patient Safety Awareness Week to encourage and create awareness among health care professionals and patients, to engage with each other to openly discuss patient safety challenges and the leading practices that can help reduce patient risk.

The activities conducted were:

  • Shared videos and picture quotes in social media on staff perception of patient safety and importance of patient safety culture in a hospital.
  • Patient forum was held whereby patients expressed their perception on patient safety and ways of improving our services were discussed
  • Had a leadership walk round, whereby management was involved
  • Patient safety workshop was held for 2 consecutive days which involved other healthcare providers from Male’, Hulhumale’, and Villimale.
  • The topics presented for the workshop were on patient safety related topics. Total 101 participants were invited and 84 participants turned up for the workshop. The overall feedback from participants showed that the sessions presented were effective and relevant to them. Whereas, 53% of participants agreed that the workshop sessions met their expectations, 43% of them said it was above average and 4% of them said it attained minimum requirement.
  • Clinical audit results of 2018 were displayed in Out Patient Department TV screens
  • Space decoration on patient safety themes were displayed by individual units and the best 3 decorations were awarded
  • All staffs adorn a badge throughout the week which says “Ask Me How I Provide Safe Care”
  • A survey on patient safety culture for staff was carried out
  • Patient safety champions were recognized and awarded in the closing ceremony of the week.
  • Participated in a complimentary webinar conducted by Institute for Healthcare Improvement (IHI), USAon Advancing Patient Safety beyond the Hospital  

Patient safety is the cornerstone of high-quality health care. In ADK hospital to maintain quality of care and improving patient safety we measure and audit eight patient safety components. Which are:

  1. SBARS audits-  to check effectiveness and proper use of SBARS communication tool
  2. Documentation audits – to check adherence to documentation by staff
  3. Hand hygiene – to check staff compliance rate to 5 moments of hand hygiene
  4. Clinical incidents – to maintain records incidents and to prevent similar incidents and reoccurrence of same incidents.
  5. Medication Administration Management audits – to check for safe medication practices across the hospital
  6. Surgical safety checklist audit – to check staff compliance on surgical safety checklist which ensure safety of surgery before & throughout surgery processes
  7. ADK Score chart audits – it is early detection of patient deterioration chart, audits are being done to check proper use of this chart and staff action according to the scoring.
  8. Health Care Associated Infections rate including -
  9. Central line associated bloodstream infection (CLABSI)
  10. Catheter Associated Urinary Tract Infection (CAUTI)
  11. Surgical Site Infection (SSI)
  12. Ventilator Associated Infection (VAP)
  13. Phlebitis
  14. Methicillin-Resistant Staphylococcus Aureus (MRSA)
  15. Multi Drug Resistant (MDR)

Patient safety is an ongoing process. ADK Hospital will ensure to take every opportunity to improve patient safety and quality of care provided to our patients with more rigorous measures.
